ATTENTION ALL STAFF

ROUTINE MANDATORY COVID TEST REQUIRED

If you have not yet been fully vaccinated, you will be required to be tested weekly for

COVID-19. Our initial test date is set for this Friday 8/27/2021, you will need to find an alternative testing site.

Please let your supervisor know if you cannot come this Friday.

WEEKLY COVID On-Site TEST ARE SCHEDULED FOR:

EVERY THURSDAY FROM 2:00 PM-3:00PM

LOCATION: Activity Room

NOTE: ALL EMPLOYEES ARE REQUIRED TO TEST AT LEAST

ONCE A WEEK UNTIL FURTHER NOTICE.

THIS IS REQUIRED BY THE STATE FOR ALL RESIDENTIAL CARE FACILITIES.
