New hire to complete and return
Direct Deposit AuthorizationIf you offer direct deposit, have new hires complete this form to have their pay deposited directly into their bank account.
I-9 Form: Employment Eligibility Verification This form must be completed on an employee’s date of hire to verify that he or she is eligible to work in the U.S. Retain for at least three years, or for one year after termination, whichever is later.

Federal W-4 (2021) This form must be completed by each employee to determine the appropriate level of federal income tax to withhold from earnings.
